Files Individually

1. RIGHT CLICK ONLY on the "Envelope" below that corresponds to the letter you wish to download. DO NOT LEFT CLICK on the envelope picture or on the descriptive text hyperlink!!
2. Click "Save Target As." This will bring up a save dialog box.
3. Select the folder/location on your computer where you would like to save the file.
4. Repeat this for each sample you wish to download OR just do it for the ZIPPED file - it contains all 3 of the samples (click here for the ZIPPED file).
5. Open and customize the file where appropriate. (PLEASE DO NOT CHANGE any of the LEGAL verbiage on the Authorization Form!)
(The Planning
Booklet is in Adobe .PDF format. The Sample Letters are formatted in MS Word version
95, which means that most versions of MS Word will be
able to read them without

4. In order to open the file from your computer, you will need to download a WinZip program (unless you already have this or some other program that "unzips" a zipped file). To download a Free Evaluation Version of WinZip 9.0, click here. (You will have to install WinZip in order for it to work).
